Short answer

Four things: no identity verification of any kind, crypto-only payment with no card or bank involved, 138 locations in 116 countries including genuine offshore jurisdictions, and an architecture that generates no connection logs. Combined with KVM-only virtualisation, included DDoS protection and open port 25 at every tier.

Most providers offer some of this. Very few offer all of it, and almost none offer it at the $4 entry price rather than as a premium privacy product.
